There are various prenups available depending on where you live.

In the US, the ORA halachic prenup is widely used and approved by the RCA
In Israel, Yad LaIsha, an organization that frees agunot and works with the rabbinic courts has created an English and Hebrew version of both a prenup and a document to prevent igun in the case of a spouse being incapacitated.
There are two different documents, one is to be signed before the wedding and one after. All information is on their website and the prenup can be found here.

In Israel, Center for Women’s Justice has a version that is both civil and halachic. Information can be found here:
The halachic prenuptial agreement is the most effective method currently available to the Jewish community to prevent get refusal. Chochmat Nashim believes that no couple should get married without signing a halachic prenup.
